Techniques for detecting infections in a patient in relation to temperature values obtained from implantable temperature sensors are described. An example implantable temperature sensor may be included within a housing of an implantable medical device (IMD). In some examples, the temperature sensor may determine a plurality of temperature values over time. Processing circuitry of the IMD or of an external device may smooth the temperature values and apply an infection detection model to the smoothened temperature signal to determine an infection status of the patient.

Techniques are disclosed for automatically calibrating a reference orientation of an implantable medical device (IMD) within a patient. In one example, sensors of an IMD sense a plurality of orientation vectors of the IMD with respect to a gravitational field. Processing circuitry of the IMD processes the plurality of orientation vectors to identify an upright vector that corresponds to an upright posture of the patient. The processing circuitry classifies the plurality of orientation vectors with respect to the upright vector to define a sagittal plane of the patient and a transverse plane of the patient. The processing circuitry determines, based on the upright vector, the sagittal plane, and the transverse plane, a reference orientation of the IMD within the patient. As the orientation of the IMD within the patient changes over time, the processing circuitry may recalibrate its reference orientation and accurately detect a posture of the patient.

A medical device is configured to produce an accelerometer signal and detect a patient fall from the accelerometer signal. The device generates a body posture signal and a body acceleration signal from the accelerometer signal and detects a patient fall in response to determining that the body posture signal and the body acceleration signal meet fall detection criteria. The medical device is configured to receive a truth signal from another device that is not the medical device. The truth signal may indicate that the detected patient fall is a falsely detected patient fall and, responsive to receiving the truth signal, the medical device adjusts at least one fall detection control parameter.

A medical device is configured to generate fluid status signal data of a patient by determining impedance metrics from an impedance signal, determining cardiac electrical signal amplitudes from a cardiac electrical signal and determining a calibration relationship between the impedance metrics and cardiac electrical signal amplitudes. The medical device generates a fluid status signal data by adjusting cardiac electrical signal amplitudes according to the determined calibration relationship. The fluid status signal data may be displayed or monitored for detecting a change in the patient's fluid status.

The exemplary systems and methods may monitor one or more signals to be used to assess the hemodynamic status of a patient. The one or more signals may be used to calculate, or determine, a plurality of pulse transit times. The plurality of pulse transit times may be used to determine hemodynamic status values that may be indicative of a patient's aggregate hemodynamic status.

A system and method for virtually detecting a medical condition, such as acute myocardial infarction (AMI), in a patient using holistic diagnostic procedures implemented in medical devices. Physiological parameters in a patient are monitored in an implantable medical device (IMD) to detect deviations from desired characteristics. When severe physiological parameter deviations exist indicating with a desired certainty that the patient is experiencing a medical condition (e.g., AMI), an alert is generated. If only minor deviations from the desired characteristics exist, additional holistic diagnostic procedures are performed virtually for diagnosing whether the patient is likely to be experiencing the medical condition, such as querying the patient through an external device regarding symptoms the patient is experiencing and analyzing the patient's responses to the questions to determine whether the patient is experiencing the medical condition. When diagnosis is still unclear, additional holistic diagnostic procedures can be performed virtually until diagnosis is certain.

A method and apparatus for detecting atrial arrhythmias include acquiring a cardiac signal comprising R-waves. Differences between pairs of consecutive R-R intervals occurring during a first time interval are computed from the cardiac signal.
