At least some embodiments of the present disclosure are directed to an electroporation ablation system for treating targeted tissue in a patient. The electroporation ablation system comprises an ablation catheter including catheter electrodes configured to generate electric fields in the targeted tissue in response to a plurality of electrical pulse sequences delivered in a plurality of therapy sections; a controller configured to receive a first pulse voltage of a first electrical pulse sequence measured during a first therapy section of the plurality of therapy sections; and determine a charge voltage based on the first pulse voltage; and an electroporation generator. The electroporation generator is operatively coupled to the catheter electrodes and the controller and configured to deliver a second electrical pulse sequence at a controlled pulse voltage for a second therapy section of the plurality of therapy sections.

At least some embodiments of the present disclosures are directed to a hybrid electroporation ablation catheter. In some embodiments, the hybrid electroporation ablation catheter comprises a catheter shaft having a proximal end and an opposite distal end and an electrode assembly extending from the distal end of the catheter shaft and the electrode assembly comprising a plurality of energy-delivering electrodes. The electrode assembly is configured to be selectively operable in a plurality of different operation modes.

A leadless pacing device may include a housing having a proximal end and a distal end, and one or more electrodes supported by the housing. The housing may include a body portion and a header. A distal extension may extend distally from the header of the housing, the distal extension including one or more electrodes. The header may include a guide wire port and a guide wire lumen may extend from the guide wire port through the header of the housing and through the distal extension. A fixation member may extend from the header of the housing. The header may be formed from an over mold process.

Systems and methods for treating cardiac arrhythmias

Disclosed herein are apparatus, systems, and methods for ablating tissue in a patient by electroporation. Embodiments generally include an ablation catheter having a hand, a shaft, and an electroporation electrode arrangement. The shaft has a distal end and defines a longitudinal axis of the ablation catheter. The electroporation electrode arrangement is at the distal end of the shaft and is configured to generate a multidirectional electric field when at least one pulse sequence is delivered thereto. The multidirectional electric field includes at least two of the following directions relative to the longitudinal axis: generally axial, circumferential, and transverse. The electroporation electrode arrangement is configured to operatively couple to an electroporation generator that is configured to generate the at least one pulse sequence and is configured to receive the at least one pulse sequence from the electroporation generator.

An implantable medical device (IMD) is configured with a pressure sensor. The IMD includes a housing and a diaphragm that is exposed to the environment outside of the housing. The diaphragm is configured to transmit a pressure from the environment outside of the housing to a piezoelectric membrane. In response, the piezoelectric membrane generates a voltage and/or a current, which is representative of a pressure change applied to the housing diaphragm. In some cases, only changes in pressure over time are used, not absolute or gauge pressures.

A ventricularly implantable medical device that includes a sensing module that is configured to detect an atrial fiducial and identify an atrial contraction based at least on part on the detected atrial fiducial. Control circuitry in the implantable medical device is configured to deliver a ventricular pacing therapy to a patient's heart based at least in part on the identified atrial contraction, and can automatically switch or revert the ventricular pacing therapies on the fly.

Implantable medical devices (IMD), such as but not limited to leadless cardiac pacemakers (LCP), subcutaneous implantable cardioverter defibrillators (SICD), transvenous implantable cardioverter defibrillators, neuro-stimulators (NS), implantable monitors (IM), may be configured to communicate with each other. In some cases, a first IMD may transmit instructions to a second IMD. In order to improve the chances of a successfully received transmission, the first IMD may transmit the instructions several times during a particular time frame, such as during a single heartbeat. If the second IMD receives the message more than once, the second IMD recognizes that the messages were redundant and acts accordingly.

Delivery devices, systems, and methods for delivering implantable leadless pacing devices are disclosed. An example delivery device may include an outer tubular member including a lumen extending from a proximal end to a distal end thereof and an intermediate tubular member including a lumen extending from a proximal end to a distal end thereof. A distal holding section may be coupled to the intermediate tubular member and define a cavity therein for receiving a proximal implantable leadless pacing device and a distal implantable leadless pacing device in a linear arrangement. The distal holding section may have a proximal body portion and a distal body portion. The proximal body portion may be more flexible than the distal body portion. An inner tubular member including a lumen extending from a proximal end to a distal end thereof may be slidably disposed within the lumen of the intermediate tubular member.

Delivery devices and methods for leadless cardiac devices

A system may include a leadless cardiac pacing device including a body, a proximal hub, and a helical fixation member opposite the proximal hub; and a first elongate shaft having a lumen extending from a distal end of the elongate shaft proximally into the elongate shaft and a transverse member extending transversely across the lumen. The proximal hub may include a transverse channel extending into the proximal hub, the transverse channel being configured to engage the transverse member.

Tools and systems for implanting and/or retrieving a leadless cardiac pacing device with helix fixation

Methods and systems for use of the Q-wave to R-wave interval to guide placement of a leadless cardiac pacemaker are disclosed. An implant delivery device is equipped with sensing electrodes to sense R-wave onset in a ventricle of a patient's heart to allow placement at a location of last or latest onset of the R-wave. Guidance tools are provided to assist in determination of the Q-wave to R-wave interval during implantation. For a chronic system, a cooperative approach is disclosed in which an implantable medical device and a leadless cardiac pacemaker exchange data to determine Q-wave to R-wave intervals and enhance cardiac resynchronization therapy delivery by the leadless cardiac pacemaker.

An implantation and/or retrieval device for a leadless cardiac pacing device may include a first elongate shaft including a lumen; a second elongate shaft slidably disposed within the lumen of the first elongate shaft; an end cap assembly fixedly attached to a distal end of the first elongate shaft; and a plurality of wires attached to the second elongate shaft and extending distally from the end cap assembly, the plurality of wires being movable relative to the end cap assembly. The plurality of wires is configured to engage a proximal hub of the leadless cardiac pacing device. The plurality of wires forms a plurality of wire loops extending distally from the end cap assembly.

Systems, methods and implantable devices configured to provide cardiac resynchronization therapy and/or bradycardia pacing therapy. A first device located in the heart of the patient is configured to receive a communication from a second device and deliver a pacing therapy in response to or in accordance with the received communication. A second device located elsewhere is configured to determine an atrial event has occurred and communicate to the first device to trigger the pacing therapy. The second device may be configured for sensing the atrial event by the use of vector selection and atrial event windowing, among other enhancements. Exception cases are discussed and handled as well.
