A system for catheter-based aspiration includes an aspiration catheter having a distal portion configured for insertion within a blood vessel of a subject, the aspiration catheter including an aspiration lumen having a distal opening at the distal portion of the aspiration catheter, an injection lumen having a distal end at the distal portion of the aspiration catheter, and an orifice located at or near the distal end of the injection lumen, the orifice configured to direct liquid into the aspiration lumen at or near the distal opening of the aspiration lumen; an injector configured to pressurize the liquid within the injection lumen, such that liquid exits the orifice and enters the aspiration lumen; and a mechanical oscillator coupled to the injector and configured to cause a pulsatile pressure of the liquid as it is injected through the injection lumen.

Systems and methods for injection and aspiration

An extension conduit for use with a system for aspirating thrombus includes a passageway extending between a distal end and a proximal end of the extension conduit, and a combined hydraulic and electrical control carried on the extension conduit and configured to be activated by a user to activate an electric switch while opening a valve to allow flow through the passageway.

Systems and methods for removal of blood and thrombotic material

A method of removing a thrombus including providing an aspiration catheter having an aspiration lumen and a supply lumen, providing a cassette configured to be connected to the supply lumen of the aspiration catheter, the cassette comprising a piston, and providing a pump base configured to receive the cassette in a movable engagement. The method also includes inserting the aspiration catheter into the vasculature of a patient, advancing the aspiration catheter to a region of interest, and operating the pump base to provide the fluid to the supply lumen of the aspiration catheter.

A system for aspirating thrombus includes a catheter having a supply lumen having a distal end and an aspiration lumen configured to couple to a vacuum source and having an interior wall surface and an open distal end, an orifice at or near the distal end of the supply lumen, in fluid communication with the interior of the aspiration lumen, the orifice located proximally of the open distal end of the aspiration lumen, wherein the orifice is configured to create a spray pattern that impinges on the interior wall surface of the aspiration lumen when a distal end of the aspiration catheter is immersed within an aqueous environment, and a disposable tubing set having a first conduit configured to couple the supply lumen of the aspiration catheter to a fluid source, and a pump component associated with the first conduit and configured to detachably couple to a drive unit.

An aspiration system includes an elongate tubular member having a lumen; an aspiration catheter configured to be inserted through the lumen of the elongate tubular member, and including a tubular aspiration member having a proximal end, a distal end, and a lumen, and configured to at least partially extend out of the lumen of the elongate tubular member at the distal end of the elongate tubular member; an elongate support member coupled to the tubular aspiration member and extending between a proximal end of the aspiration catheter and the proximal end of the tubular aspiration member; and an annular sealing member coupled to the tubular aspiration member and configured to create a seal against an inner surface of the elongate tubular member, when a vacuum sufficient to cause aspiration is actively applied to the lumen of the elongate tubular member.

Systems and methods for management of thrombosis

A system for aspiration includes an aspiration catheter including an elongate shaft including an aspiration lumen, and an open distal end, an extension tube, the lumen of the extension tube configured to be hydraulically coupled to the aspiration lumen, a peristaltic pump configured for driving fluid through the extension tube, a compressible portion disposed between the distal end and the proximal end of the extension tube, the compressible portion configured to be coupled to the peristaltic pump, such that operation of the peristaltic pump drives fluid from the aspiration lumen from the distal end of the extension tube to the proximal end of the extension tube, and a controller configured to receive a first signal from the first sensor and configured to vary the operation of the peristaltic pump based at least in part on a first signal received from a first sensor coupled to the aspiration lumen or extension tube related to a change in the characteristic of flow.

Systems and methods for aspiration and monitoring

A system for removal of blood or thrombus includes an aspiration catheter having an elongate shaft including an aspiration lumen having proximal end configured to couple to a vacuum source, and a distal end having an orifice, an elongate member configured for placement through the aspiration lumen and having a distal portion including a disruption element configured to disrupt thrombus within the aspiration lumen, and a monitoring device configured for removable connection in between the aspiration catheter and the vacuum source, and including a housing, a pressure sensor in fluid communication with an interior of the housing, a measurement device coupled to the pressure sensor and configured for measuring deviations in fluid pressure, and a communication device coupled to the measurement device and configured to generate an alert signal when a deviation in fluid pressure measured by the measurement device exceeds a pre-set threshold.

Aspiration monitoring system and method

A system for real time monitoring of catheter aspiration includes a pressure sensor configured for placement in fluid communication with a lumen which at least partially includes an aspiration lumen of a catheter, the aspiration lumen configured to couple to a vacuum source, a measurement device coupled to the pressure sensor and configured for measuring deviations in fluid pressure, and a communication device coupled to the measurement device and configured to generate a continuous signal which is proportional to measured fluid pressure.

In one embodiment, system for real time monitoring of catheter aspiration includes a housing having a first port adapted for connection to a vacuum source and a second port adapted for connection with an aspiration catheter, a pressure sensor in fluid communication with an interior of the housing, a measurement device coupled to the pressure sensor and configured for measuring deviations in fluid pressure, and a communication device coupled to the measurement device and configured to generate an alert signal when a deviation in fluid pressure measured by the measurement device exceeds a pre-set threshold. In another embodiment, the system for real time monitoring of catheter aspiration further includes a vacuum source for connection to the first port and an aspiration catheter having an aspiration lumen for connection to the second port.

A method for improving a flow condition through a catheter inserting a distal end of a sheath within the vasculature of a subject, placing the aspiration catheter through the sheath and advancing the aspiration catheter such that an open distal end of the aspiration lumen is distal to a distal end of the sheath and is in proximity to a thrombus within a blood vessel of a subject, coupling an extension conduit in fluid communication with a lumen of the sheath to a second fluid source, and activating a pump such that pressurized fluid from the first fluid source is applied to the supply lumen of the aspiration catheter, wherein when an active flowing condition is not present, fluid from the second fluid source is caused to flow through the lumen of the sheath.

A method for treating a patient having thrombus includes providing a manipulation device including a housing configured to be supported by the hand of a user and having a distal and proximal end, a drive system disposed within the housing, and configured to rotate a rotation member, an engagement member coupled to the rotation member and configured to be removably coupled to an elongate medical device, an activation member carried by the housing such that it can be operated by at least a portion of the hand of the user when the housing is supported by the hand of the user, the drive system configured to apply motive force to the engagement member, securing an elongate member to the engagement member, introducing at least the distal end of the elongate member into a blood vessel adjacent a thrombus, operating the activation member to cause at least some rotation of the rotation member, which in turn causes at least some rotation of the distal end of the elongate member at or near the thrombus, and aspirating at least some of the thrombus with an aspiration catheter.

Systems and methods for manipulating medical devices

An aspiration system includes an aspiration catheter including a tubular aspiration member having a proximal end, a distal end, and a lumen, and configured to at least partially extend out of the lumen of an elongate tubular member and into the vasculature of a subject, an elongate support member coupled to the tubular aspiration member and extending between the proximal end of the aspiration catheter and the proximal end of the tubular aspiration member, a high pressure injection lumen extending within the elongate support member and having a distal end including a curved portion configured to change a direction of fluid flow by at least about 90&#xb0;, at least one orifice located at the distal end of the high pressure injection lumen configured to allow liquid to be released into the lumen of the tubular aspiration member, and an annular seal carried by the tubular aspiration member and configured to create a liquid seal against the inner surface of the elongate tubular member.

A system for aspirating thrombus and delivering an agent includes an aspiration catheter having a supply lumen having a proximal end, a distal end, and a wall, and an aspiration lumen having a proximal end, an open distal end, and an interior wall surface adjacent the open distal end, and at least one orifice at or adjacent the distal end of the supply lumen, in fluid communication with the aspiration lumen and located proximally of the open distal end of the aspiration lumen, wherein the at least one orifice is configured to create a spray pattern that is caused to impinge on the interior wall surface of the aspiration lumen such that the spray pattern upon impinging on the interior wall surface is caused to transform into at least a substantially distally-oriented flow capable of exiting the open distal end of the aspiration lumen.
