Systems, devices, and methods for electroporation ablation therapy are disclosed herein, including an inflatable member for positioning an ablation device within a pulmonary vein ostium. An apparatus can include first and second shafts moveable relative to one another, first and second electrodes configured to generate an electric field for ablating tissue, and an inflatable member disposed between the first and second electrodes. In some embodiments, the inflatable member is configured to transition from an undeployed configuration to a deployed configuration in response to movement of the first and second shafts. In some embodiments, the inflatable member in the deployed configuration can engage a wall of a pulmonary vein ostium and direct the electric field generated by the first and second electrodes toward the wall.

The present invention may include a medical device having a handle, a catheter coupled to the handle, and an expandable element coupled to the catheter. The medical device may also include first and second elongate bodies that traverse a length of the handle and the catheter. A housing may be disposed within the handle, with the housing defining a first opening able to receive a portion of the first elongate body, a second opening able to receive a portion of the second elongate body, and a third opening opposite the first and second openings able to receive a portion of both the first and second elongate bodies. A separation element may be disposed within the housing, with the separation element defining a path able to receive a portion of the second elongate body, and whereby a portion of the first elongate body forms a loop around the separation element.

A device and method for cryosurgical ablation. A tip has a thermally transmissive region along a length thereof in which the thermally transmissive region is operable at a temperature sufficient to cryosurgically ablate tissue in contact therewith. A plurality of cryosurgical fluid injection lumens each have a first end positioned within the tip at a different point along the length of the thermally transmissive region. Each of the first ends is arranged to cool overlapping respective portions along the length of the thermally transmissive region when cryogenic fluid is ejected from the plurality of cryogenic fluid injection lumens.
