A surgical instrument and related method includes an instrument body, a guide shaft distally projecting from the instrument body, and a first surgical tool. The guide shaft has a guide sidewall, a guide lumen, and a clearance opening radially extending through the guide sidewall in communication with the guide lumen. The first surgical tool has an elongate body and a distal head configured to deflect relative to the elongate body from a first position to a second position. The distal head in the first position is positioned within the guide lumen along the central axis. The distal head in the second position is deflected from the central axis and extends at least partially through the clearance opening thereby vacating at least a portion of the guide lumen for introducing a second surgical tool through the guide lumen.

Instrument with integral imaging and irrigation features

A guidewire includes a guidewire shaft and an inflatable element arranged at a distal end of the guidewire shaft. At least a portion of the guidewire shaft is formed of metal, and the guidewire shaft includes a lumen that fluidly communicates with an interior of the inflatable element. The inflatable element is operable to transition between a deflated state in which the inflatable element is configured to pass through an isthmus of a Eustachian tube (ET), and an inflated state in which the inflatable element is configured to dilate the ET.

A surgical instrument includes a handle assembly and a shaft assembly extending distally from the handle assembly and having a distal end sized to be inserted into the nasal cavity of a patient. The shaft assembly includes a cutting member configured to cut tissue within the nasal cavity, and a translating member slidably disposed over the cutting member. A navigation sensor is disposed within the distal end of the shaft assembly and is operable to generate a signal corresponding to a position of the distal end within the patient.

A virtual camera may be positioned relative to a 3-D model of a patient anatomy in order to provide a virtual camera view of the surrounding anatomy and tracked surgical instruments being deployed to the anatomy. Visual context provided by the virtual camera may be limited in some cases, such as where a surgical instrument is being used within a very narrow anatomical passageway or cavity. To provide more placement flexibility, an IGS system providing a virtual camera receives inputs defining variable visual characteristics for different segments or regions of the 3-D model, which may include hiding certain segments or making certain segments semi-transparent. With such a system, the view of the 3-D model provided by the virtual camera view may be modified to remove or deemphasize less relevant segments, to display or emphasize more relevant segments (e.g., critical patient anatomy), or both.

User interface for image guided surgery system

A method of dilating a target anatomical structure of an ear, a nose, or a throat of a patient includes inserting a distal end of a balloon catheter into the ear, the nose, or the throat. The balloon catheter includes a balloon assembly coaxially disposed along a proximally extending shaft. The balloon assembly includes at least first and second dilation balloons that at least partially overlap one another. The method also includes distally advancing the distal end until the balloon assembly is disposed in the target anatomical structure. The method also includes selectively dilating the target anatomical structure based on a diameter of the target anatomical structure by inflating the first dilation balloon to a first diameter or inflating the second dilation balloon to a second diameter that is greater than the first diameter. The second inflation lumen is not in fluid communication with the first inflation lumen.

An apparatus includes a body, a first shaft, an actuation assembly, and a dilation catheter. The first shaft includes a malleable distal portion. The actuation assembly includes a second shaft and an actuator. The second shaft is coaxially positioned about the first shaft. The actuator is operable to selectively bend the malleable distal portion of the first shaft. The dilation catheter is coaxially interposed between the first shaft and the second shaft. The dilation catheter includes an expandable dilator. The dilation catheter is operable to translate along the malleable distal portion of the first shaft.

A virtual endoscopic view shows a surgical area and surrounding anatomy and may also show a position of a surgical instrument in use during a surgical procedure, allowing a surgeon to virtually view the surgical area when direct viewing or actual endoscopic views are incomplete, obstructed, or otherwise unavailable or undesirable. In order to render the endoscopic view, an IGS navigation system may be configured with an observer point and an observer orientation within 3-D space based upon user inputs. A user interface for defining these points allows a user to view a virtual endoscopic preview in real-time while providing inputs, thus improving the likelihood that the resulting virtual endoscopic view is as desired by the user; and reducing time spent redefining and reconfiguring the virtual endoscopic view. The virtual endoscopic preview may provide combinations of static and dynamic images to illustrate the spatial relationship of the provided inputs.

Method for real time update of fly-through camera placement

A surgical instrument includes an elongate shaft having a proximal shaft portion and a malleable distal shaft portion. The elongate shaft is configured to be secured to a supporting surgical instrument. An ablation head is coupled to the malleable distal shaft portion and includes at least one electrode operable to deliver RF energy to tissue for ablating the tissue. The ablation head is sized to fit within the nasal cavity of a patient with a distal end of the supporting surgical instrument. The proximal shaft portion is configured to operatively couple with an RF energy source operable to energize the at least one electrode with RF energy. The malleable distal shaft portion is configured to bend relative to a longitudinal shaft axis defined by the proximal shaft portion for selectively orienting the ablation head relative to the longitudinal shaft axis.

Instrument for endoscopic posterior nasal nerve ablation

A method includes inserting a first dilation catheter into a first nostril of a patient. A first dilator of the first dilation catheter is positioned between the nasal septum of the patient and the turbinate of the patient. The first dilator is expanded, thereby remodeling one or more of the nasal septum, the turbinate, or mucosal tissue of the patient. The first dilation catheter is removed from the nostril of the patient. A second dilation catheter may be inserted into a second nostril of the patient. A dilator of the second dilation catheter may provide an opposing force on the nasal septum to prevent over-medialization of the nasal septum.

Method of treating deviated nasal septum, enlarged nasal turbinate, or mucosal hypertrophy

An apparatus may be used with an elongate member that includes an engagement member, a hollow shaft, a compression member, a compression element, and a resilient gripping member. The hollow shaft extends from the engagement member along a longitudinal axis. The compression member is slidably disposed over the hollow shaft. The hollow shaft is disposed over the resilient gripping member and supports the compression element. The compression member has a conical inner surface including a detent feature. The conical inner surface is configured to urge the compression element radially inwards as the collar advances from the unlocked position toward the locked position where the detent feature retains the compression element. The compression element deforms the resilient gripping member radially inwards to grip the elongate member and prevents translation of the elongate member. When unlocked, the resilient gripping member moves radially away from the elongate member allowing translation.

Coupling assembly for variable diameter surgical instrument

A surgical instrument includes an outer tube configured to be gripped by a user, and a needle slidably disposed within the outer tube. The needle includes a needle lumen, a distal needle tip configured to pierce tissue, and an electrode disposed at the distal needle tip. The needle lumen opens to the distal needle tip such that the distal needle tip is configured to deliver fluid from the needle lumen to tissue. The electrode is operable to deliver RF energy to tissue for ablating the tissue. The needle is translatable relative to the outer tube between a proximal retracted position in which the distal needle tip is housed coaxially within the outer tube, and a distal extended position in which the distal needle tip is exposed from the outer tube and configured to pierce tissue.

An instrument includes a handle assembly, a shaft, a dilation catheter, and a position sensor. The shaft extends distally from the handle assembly. The shaft includes a distal end that is configured to be introduced into an anatomical passageway within a head of a human. The dilation catheter includes an expandable dilator and is configured to advance distally relative to the shaft. The position sensor is configured to generate signals indicating a position of the position sensor within the head. The position sensor is configured to advance distally between at least first and second positions. The position sensor is disposed adjacent the distal end of the shaft in the first position, and is configured to be carried further distally by the dilation catheter to the second position. In the second position, the position sensor is separated a distance from the distal end of the shaft.
