A vial adaptor () may comprise a body portion (). The body portion includes a vial connection port (), a syringe connection port (), an access passageway () between the vial connection port and the syringe connection port, and a regulation passageway (). The vial adaptor may further comprise an expandable and/or contractible chamber () impermeable to gas and/or liquid, the regulation passageway being between the vial connection port and the chamber, and an expandable housing () casing the chamber. Such a vial adaptor constitutes an improved vial adaptor.

A fluid transfer device () includes a vial holder () configured to hold a vial (). An actuator () is coupled to the vial holder () and can rotate the vial () between upright and inverted positions. A pump (positive or negative pressure) can pump contents out of the vial (). Before pumping of the contents the actuator () rotates the vial () to the inverted position and after pumping of the contents the actuator () rotates the vial () to the upright position.
