An apparatus includes a handle assembly having a body, a shaft assembly having an outer tube member, an end effector, an anvil, a first trocar closure system, and a second trocar closure system. The end effector has having a staple deck fixed relative to the body, a staple driver operable to actuate relative to the staple deck between an unfired position and a fired position, and the trocar configured to selectively couple with the anvil and actuate relative to the staple dreck. The first trocar closure assembly can actuate in a first motion to drive the trocar between a distal position and a first closed position. The second trocar closure assembly can actuate in a second motion between the first closed position and a second closed position. The staple drive is inoperable at least until the first trocar closure assembly actuates the trocar to the first closed position.

Methods and devices reroute chyme to induce intestinal brake in order to improve the effectiveness of bariatric surgical procedures and to improve comorbidity resolution. A bowel is manipulated to provide a shortened path for chyme to travel to the ileum. These methods and devices of rerouting chyme to induce intestinal brake may include one or more of a surgical procedure, an implanted device, or a combination of an implant with an improved surgical procedure.

Methods and devices for the rerouting of chyme to induce intestinal brake

A surgical clip applier and methods for applying surgical clips to a vessel, duct, shunt, etc., during a surgical procedure are provided. In one embodiment, a surgical clip applier is provided and can include a housing having a trigger movably coupled thereto and an elongate shaft extending therefrom with opposed jaws formed on a distal end thereof. The clip applier can include an advancer assembly disposed within the elongate shaft and configured to advance one of a plurality of clips disposed within the elongate shaft into the opposed jaws. A feeder shoe can be disposed within the elongate shaft and can be configured to engage and prevent the advancer assembly from moving to a proximal position after the advancer assembly has moved to a distal position to advance a proximal-most clip into the opposed jaws. This can indicate to a user that a clip supply of the surgical clip applier is depleted.

A retractor for use in laparoscopic procedures includes an upper retractor ring, a lower retractor ring, and a retractor sheath extending between the upper retractor ring and the lower retractor ring to form a tubular passageway through which instruments or a medical practitioner's hands may pass during a medical procedure. The retractor sheath includes an elliptical passageway extending between lower retractor ring and the upper retractor ring.

A surgical clip applier and methods for applying surgical clips to a vessel, duct, shunt, etc., during a surgical procedure are provided. In one embodiment, a surgical clip applier is provided and can include a shaft having a proximal end and a distal end with opposed jaws thereon, a guide member disposed within the shaft and configured to guide a clip into the opposed jaws, the guide member having a channel formed in a surface thereof, and an advancer movably disposed within the shaft and configured to advance a clip over the guide member and into the opposed jaws, the advancer having a distal tip that slidably engages the channel for maintaining contact with a surgical clip as it is advanced into the opposed jaws. In other embodiments, the guide member can also include a proximal channel formed in a superior surface thereof. The advancer can be configured to deflect downward into the proximal channel to move proximally beneath an inferior surface of the apex of the clip to position itself proximally to the clip to advance the clip into the opposed jaws.

A laparoscopic surgical access device comprises a first frame and a second frame having relative axial positions. A flexible and resilient iris valve is connected to the first and second frames and has a closed position and an opened position based on the relative angular position of the first and second frames. A lock is connected to the first and second frames, the lock being selectively operative to prevent relative rotational movement in a first rotational direction. One embodiment uses a ratchet and pawl mechanism operable with a control ring. Another embodiment uses a ratchet and pawl mechanism operable with a button. Yet another embodiment uses a wedge track and ball mechanism.

A seal assembly for permitting hand assisted laparoscopic procedures includes a seal cap having a central access opening allowing access to a body cavity as desired. The seal cap is provided with a housing in which a seal is positioned. The seal cap also includes an attachment ring that is selectively detachable therefrom for selective attachment of a retractor.

A surgical instrument adapted for applying a plurality of surgical fasteners to body tissue includes a frame having a proximal end and a distal end, with a handle positioned at the proximal end. The instrument also includes a cartridge housing and an anvil. The cartridge housing and anvil are relatively movable between a first spaced apart position and a second position in close approximation with one another, and the cartridge housing includes a knife adapted for cutting tissue by moving between the cartridge housing and the anvil. A firing mechanism is associated with the cartridge housing for selective actuation. The instrument also includes a tissue retention feature having an upper member and a lower member maintaining tissue between the cartridge housing and the anvil during treatment, wherein the upper and lower members respectively include channels associated with the knife for guiding it between the cartridge housing and the anvil.

A method is provided for removing at least one tissue sample from a surgical patient using a biopsy instrument. The biopsy instrument comprises an elongated piercer having a piercer lumen extending therethrough. The biopsy instrument further comprises a cutter rotatably and translationally positionable relative to the piercer. The piercer has a port for receiving and transferring the tissue portion into the piercer lumen. In one embodiment of the present invention, the method for using the biopsy instrument comprises the steps of engaging tissue in the port; translating the cutter at a first, predetermined translational speed from a first position to a second position proximal to the port; measuring the translational speed of the cutter; translating the cutter at a second, predetermined translational speed from the second position to a third position proximal to the port; translating the cutter at a third, predetermined translational speed from the third position to a fourth position distal to the port; rotating the cutter at a predetermined rotation speed; measuring the rotational speed of the cutter; and modifying the translational speed of the cutter when the rotational speed of the cutter varies from the predetermined rotational speed by more than a first, predetermined differential rotational speed.

Control method for an automated surgical biopsy device

A hand held surgical instrument for grasping and clamping tissue is disclosed. The surgical instrument has a handle body assembly and an elongated shaft. The shaft is connected to the handle body assembly at a proximal end and has an end effector at a distal end. The end effector is operably coupled to an actuation member extending from the handle body. The actuation member is moveable toward and away from the handle body to open and close the end effector. A locking tab is located on the handle body assembly and is moveable between a locked position and an unlocked position. A locking rod is movably engaged with a hole in the locking tab. When the locking tab is in the unlocked position, the actuation member is free to move towards and away from the handle body assembly to open and close the end effector. When the locking tab is in the locked position, movement to open the actuation member is prevented. The handle body assembly also has release trigger extending from the handle body assembly and operably coupled to the locking tab. The release trigger is moveable from a first position spaced away from the handle body to a second position adjacent to the handle body. The release trigger is normally biased toward the first position and is reciprocable, when initially squeezed toward the handle body assembly from the first position, to the second position, and subsequently released from the second position back to the first position. Consecutive reciprocations of the release trigger alternates the locking tab between the locked position and the unlocked position.

The present invention is a control apparatus for removing at least one tissue sample from a surgical patient using a surgical biopsy device. The biopsy device comprises an elongated piercer having a piercer lumen extending therethrough, and a cutter rotatably and axially positionable relative to the piercer. The piercer has a port for receiving and transferring the tissue sample into the piercer lumen. The biopsy device further comprises a rotation motor for rotating the cutter and a translation motor for translating the cutter in the axial direction. In one embodiment of the present invention, the control apparatus comprises a computing device for the coordinated control of the rotation and translation motors. The control apparatus further comprises a first sensor operationally connected to the rotation motor for supplying a rotation signal to the computing device for measuring the rotational speed of the cutter. The control apparatus further comprises a second sensor operationally connected to the translation motor for supplying a translation signal to the computing device for measuring the translational position and translational speed of the cutter. The control apparatus further comprises a first driver for driving the translation motor in response to a translation command from the computing device, and a second driver for driving the rotation motor in response to a rotation command from the computing device. The control apparatus automatically modifies the rotational speed and the translational speed of the cutter in response to the translational position and rotational resistance acting on the cutter.
