The present disclosure provides a device and methods for acquiring tissue within a lumen or for closure of bodily openings.

Method and device for tissue acquisition or closure

The present invention is directed to a method of endoscopically ablating/resecting/excluding (A/R/E) the mucosa of the gastric fundus and body. The method of the present invention is intended to cause cellular death to the X/A-like, ghrelin producing and other hormone cells. This is in contrast to present methods to provide only transient ischemia. The use of the method of the present invention results in a more permanent and robust method of reducing ghrelin and other hormone levels. In addition, a method according to the present invention can be used to induce scarring of the gastric fundus which will reduce gastric accommodation of excess food and result in early satiety.

Endoscopic gastric mucosal ablation/resection/exclusion (A/R/E) as a minimally invasive weight loss approach

The present invention provides a low-risk, unobtrusive and noninvasive method and device for treatment of obesity and eating disorders. In embodiments, the device is a gastric device suitable for placement in a stomach of a subject. The device may be composed of a sponge material which absorbs fluid upon implantation and expands in volume, thereby functioning as a space occupying device in the stomach to cause early satiety.

Gastric device and method of use thereof

The present invention utilizes tetherless microtools to biopsy tissue. The invention provides a device and method for deployment and retrieval of tetherless microtools. The size of the microtools ensures that tissue damage at a site targeted for biopsy is negligible. As such, large numbers of microtools may be deployed ensuring that a true statistical sampling of biologic tissue is performed.

Method and device for statistical tissue sampling using microdevices

The present invention provides an endoscopic abrasion device and methods of use thereof. The device includes a catheter having an inflation balloon including one or more elements disposed on the balloon for abrasion of tissue when the balloon is rotated about the longitudinal axis of the catheter. The device may be used to treat and diagnose diseases of the esophageal passage, such as Barrett's disease.

An implantation device for releasably holding implantable microstimulators can be used to guide one or more microstimulators to any site within the gastrointestinal tract for implantation. The device can further releasably hold one or more ligation clips for securing the one or more microstimulators in place within the implantation site(s) and/or for closing an incision in which a microstimulator is implanted. The device can be employed using open, laparoscopic, and endoscopic techniques.

Devices and methods for treatment of gastrointestinal disorders

A novel approach to diagnostic and therapeutic interventions in the peritoneal cavity is described. More specifically, a technique for accessing the peritoneal cavity via the wall of the digestive tract is provided so that examination of and/or a surgical procedure in the peritoneal cavity can be conducted via the wall of the digestive tract with the use of a flexible endoscope. As presently proposed, the technique is particularly adapted to transgastric peritoneoscopy. However, access in addition or in the alternative through the intestinal wall is contemplated and described as well. Transgastric and/or transintestinal peritoneoscopy will have an excellent cosmetic result as there are no incisions in the abdominal wall and no potential for visible post-surgical scars or hernias.

Methods for diagnostic and therapeutic interventions in the peritoneal cavity

An adjustable size balloon at the distal end of a feeding tube is used to aid in positioning, along with an endoscope, the tube in the jejunum or small bowel of a patient, without creating an obstruction in the intestines. The balloon is fully inflated when the endoscope that is used to place the tube in the duodenum is withdrawn, and then partially deflated to a size that allows peristaltic action on the balloon to move the tube into the jejunum, after which the balloon is further deflated to avoid creating an obstruction in the intestine.

A technique is described for reducing the stomach cavity endoscopically, from within the stomach or, in the alternative, from within the peritoneal cavity via a wall of the digestive tract. This new approach for reducing gastric capacity uses a flexible endoscope and a specially adapted ligating loop that is secured at spaced locations about an inner periphery of the stomach. After attachment, the loop is constricted to draw together the associated gastric wall portions to reduce the food receiving cavity defined at the base of the esophagus. The ligating loop is preferably a slip knotted loop.

Gastric reduction endoscopy

A cryoprobe instrument for endoscopic use has been provided that permits the delivery of liquid or gaseous cryogens in the form of a spray. In an embodiment, the cryoprobe instrument includes an elongated, flexible tube having a fluid passage defined therethrough; a proximal connector portion provided at the proximal end of the tube for coupling the tube to a pressurized source of cryogenic refrigerant; and a nozzle tip mounted to the distal end of the tube. The nozzle tip has an outlet orifice constructed and arranged such that through the Joule-Thompson effect, cryogenic refrigerant exiting through the outlet orifice rapidly and substantially expands and cools.

Endoscopic cryospray device

A stent, for example for maintaining the patency of a duct such as a biliary duct, is provided that includes a plurality of radially extending wings. The wings project from a core which has a guide wire or wire stylet lumen. Each of the wings preferably includes a plurality of minute, longitudinally extending grooves for bile flow even in the event of tumor ingrowth. Advantageously the core itself is preformed into a helical configuration.

Winged biliary stent

Direct injection of sphincteric botulinum toxin is disclosed as an effective, safe and simple method of treatment for disorders of gastrointestinal muscle or smooth muscles elsewhere in the body, with results that appear to be sustained for several months. Muscle disorders which are suitable for such treatment include achalasia, isolated disorders of the lower esophageal sphincter, gastroparesis, hypertrophic pyloric stenosis, sphincter of Oddi dysfunction, short-segment Hirschsprung's, anal fissure, hemorrhoids, proctalgia fugax, irritable bowel syndrome, disorders of the upper esophageal sphincter, vasospastic disorders, and disorders of uterine and bladder spasm. Devices suitable for delivering this therapy are also disclosed.
