This disclosure provides tools and implant techniques utilizing such tools to gain access to and implant a medical device, such as a medical electrical lead, within extravascular spaces. In one example, this disclosure provides a tool for creating a sub-sternal tunnel in a patient. The tool comprises a relatively straight guide member extending from a first end thereof to a second end thereof, a tunneling member extending from a first end thereof to a tip thereof, the tunneling member extending alongside and coplanar with the guide member, the first end of the tunneling member and the first end of the guide member being joined together, and a handle coupled to the guide member.

A method and device for reducing paravalvular leakage upon implantation of a replacement heart valve is provided. The valve assembly includes a tissue or bioprosthetic heart valve attached to an anchoring structure. The anchoring structure includes an inlet rim that is substantially C-shaped in cross section to form a concave landing zone. The anchoring structure self-seats when implanted in the sinus of a patient with the proximal and distal ends of the C-shaped inlet rim pushed against the aorta to effectively prevent paravalvular leakage.

A variable hemostasis valve is disclosed that may be used during medical procedures to prevent blood loss while permitting the percutaneous introduction, operation, and removal of medical instruments. A flexible tubular seal element is disclosed that is disposed within the variable hemostasis valve to provide a fluid-tight, adjustable seal. After insertion of a medical instrument into the valve through at least a portion of the flexible tubular seal element, a proximal portion of the valve housing may be rotated to cause the tension of the flexible tubular seal element to increase over the inserted medical instrument and engage the valve in a closed position. The proximal portion of the valve housing may then be rotated in an opposite direction to release the seal and return the tubular seal element to a relaxed lumen open position.

Variable hemostasis valve and method of use

A cryoprobe clamp for creating a lesion in a target tissue comprises a housing portion, a handle extending from the housing portion, a clamp assembly including a distal blade and a proximal blade structured for clamping a target tissue therebetween, a trigger mechanism positioned adjacent the handle and operably coupled to the clamp assembly, and an ablation tool extending between the housing portion and the clamp assembly. The distal blade includes receiving means structured to receive a distal portion of the ablation tool, and the proximal blade includes an outer frame surrounding an open window portion. Upon clamping the target tissue between the distal and proximal blades, a lesion formed by the distal end of the ablation tool is visible through the window portion of the proximal blade.
