A method for ablating tissue in a subject, including providing a probe, having an electrode, that is inserted into a lumen of the subject, so that the electrode is in proximity to the tissue to be ablated. An immobilizing signal is injected into the subject, via the electrode, that immobilizes the subject. When the subject is immobilized, an ablation signal is injected via the electrode into the subject, the ablation signal being configured to ablate the tissue of the subject by irreversible electroporation. The ablation signal has at least one train of first pulses, each of the first pulses in the at least one train having a first pulse absolute amplitude, and the immobilizing signal has one or more trains of second pulses, each of the second pulses in the one or more trains having a second pulse absolute amplitude less than the first pulse absolute amplitude.

Initiating IRE generation with a ramp

Apparatus, including a catheter configured to be inserted into an organ of a human body. A plurality of electrodes are deployed on the catheter, the electrodes being configured to transfer radiofrequency (RF) ablation energy to tissue of the organ. The apparatus also includes a power supply configured to supply the RF ablation energy at a level of up to 100 W to each of the plurality of electrodes simultaneously, so as to ablate respective sections of the tissue of the organ in contact with the electrodes.

Temperature controlled short duration ablation with multiple electrodes

A medical probe includes a shaft and a distal-end assembly. The shaft is configured for insertion into an organ of a body. The distal-end assembly is fitted at a distal end of the shaft. The distal-end assembly includes (a) a substrate, (b) a two-dimensional (2D) ultrasound transducer array located on the substrate, and (c) a sensor, which is also located on the substrate, the sensor configured to output signals indicative of a position and an orientation of the 2D ultrasound transducer array inside the organ.

Magnetic location sensor and ultrasound array on printed-circuit-board (PCB) of catheter and calibration thereof

A method includes emitting an ultrasound beam from an array of ultrasound transducers in a catheter placed in a blood pool in an organ. Echo signals reflected in response to the ultrasound beam are received in the array. Distinction is made in the echo signals between (i) first spectral signal components having Doppler shifts characteristic of blood and (ii) second spectral signal components having Doppler shifts characteristic of tissue of the organ. The first spectral signal components are suppressed relative to the second spectral signal components in the echo signals. An ultrasound image of at least a portion of the organ is reconstructed from the echo signals having the suppressed first spectral signal components. The reconstructed image is displayed to a user.

Ultrasound imaging of cardiac anatomy using doppler analysis

A catheter includes: (a) a shaft for insertion into a heart of a patient, (b) an expandable distal-end assembly, which is coupled to the shaft and is configured to make contact with tissue of the heart, (c) at least first and second electrocardiogram (ECG) electrodes, which are coupled to an outer surface of the expandable distal-end assembly, and when placed in contact with the tissue, are configured to sense ECG signals in the tissue, and (d) a reference electrode, which is positioned within an inner volume of the distal-end assembly, and in an expanded position of the distal-end assembly, the reference electrode: (i) has no physical contact with the tissue, and (ii) is positioned at a first distance from the first ECG electrode and at a second distance from the second ECG electrode, and the difference between the first and second distances is smaller than a predefined threshold.

Electro-anatomical mapping and annotation presented in electrophysiological procedures

An irreversible electroporation (IRE) method includes placing multiple electrodes of a catheter in contact with tissue of an organ. Bipolar IRE pulses are generated. The tissue is ablated by applying the bipolar IRE pulses to pairs of the electrodes, in accordance with an order in which successive activations of a given electrode-pair are interleaved with activation of at least one other electrode-pair, and are spaced in time by at least a predefined duration.

Sequential activation of electrode-pairs during irreversible electroporation (IRE)

A medical apparatus includes a probe, which includes an insertion tube configured for insertion into a body cavity of a patient, and a distal assembly, which is connected distally to the insertion tube and includes a plurality of electrodes, which are configured to contact tissue within the body cavity. The apparatus further includes an electrical signal generator, which is configured to apply electrical pulses to a group of two or more of the electrodes with an amplitude sufficient to irreversibly electroporate the tissue contacted by the electrodes in the group, and a controller, which is coupled to measure a change in an electrical impedance between an electrode in the group and a further electrode as a result of application of the electrical pulses and to output a measure of ablation of the tissue responsively to the measured change in the electrical impedance.

Medical systems and methods are provided in which a processor receives the 3D ultrasound images and receives position signals from a position tracking device, indicating the position of a corresponding probe relative to the 3D ultrasound images. Based on the probe position, a region of interest is selected that contains the position the medical probe within the 3D ultrasound images, and the selected region of interest is rendered to a display together with a representation of the medical probe superimposed on the region of interest.

The processor is configured to: (i) produce an electroanatomic (EA) map of a surface of the organ, the EA map specifying a propagation vector-field (PVF) indicative of propagation of an electrophysiological (EP) wave over at least the surface, (ii) select at least a region of the organ, (iii) calculate a gaussian integral of the PVF along a perimeter of at least a closed contour indicative of the selected region, and (iii) produce at the region, a tag indicative of the LAS, in case the calculated gaussian integral meets a criterion. The display is configured to display the EA map and at least the tag over the EA map.

Detecting local activation source in atrial fibrillation

A catheter includes an insertion tube, a flexible substrate and one or more electrical devices. The insertion tube is configured for insertion into a patient body. The flexible substrate is configured to wrap around a distal end of the insertion tube and includes electrical interconnections. The electrical devices are coupled to the flexible substrate and are connected to the electrical interconnections.

A system includes a processor to receive multiple electrophysiological (EP) signals acquired by multiple electrodes of a multi-electrode catheter that are in contact with tissue in a region of a cardiac chamber, and respective tissue locations at which the electrodes acquired the EP signals. The processor calculates local activation time (LAT) values for the respective tissue locations, and compares the average LAT values to determine representative locations. Based on a comparison between average LAT values the processor produces a propagation vector indicative of a propagation of an EP wave that has generated the EP signals and displays the propagation vector to a user.

Mapping resolution of electrophysiological (EP) wave propagating on the surface of patient heart

A method, including receiving, from electrodes positioned within a heart, first signals from at least three of the electrodes indicating electrical activity in tissue with which the at least three of the electrodes engage, and second signals indicating locations of the at least three electrodes. The second signals are processed to compute the locations of the at least three electrodes and to determine a geometric center of the locations. Based on the signals, an electroanatomical map is generated for an area of the tissue including the geometric center, and an arrhythmia focus is determined in the map. A circle is presented, and within the circle, a region of the map is presented including the geometric center and the focus so that the geometric center on the map aligns with a center of the circle, the region within the circle indicating a spatial relationship between the geometric center and the focus.

Visual guidance for positioning a distal end of a medical probe

An electroporation ablation system includes a probe to be inserted into a body part of a living subject, and including a distal end including at least one electrode, body-surface patches to be applied to a skin surface, an ablation power generator to apply at least one first electrical pulse train between the electrode(s) and first one(s) of the body-surface patches, and a processor to provide a measurement of movement of the living subject responsively to applying the first electrical pulse train(s) between the electrode(s) and the first one(s) of the body-surface patches, and select second one(s) of the body-surface patches responsively to the measurement of movement, and wherein the ablation power generator is configured to apply at least one second electrical pulse train between the electrode(s) and the second one(s) of the body-surface patches.

High frequency unipolar electroporation ablation

A pulsed field ablation (PFA) system includes a composite electrode, a body-surface electrode, a PFA generator, and a processor. The composite electrode is coupled to a distal end of a catheter configured for insertion into an organ of a patient. The body-surface electrode is configured to be attached to a skin of the patient. The PFA generator is configured to be electrically connected to the composite electrode of the catheter and to the body-surface electrode, and to generate Direct-Current (DC) PFA pulses. The processor is configured to control the PFA generator to apply the DC PFA pulses between the composite electrode and the body-surface electrode while the composite electrode is placed in contact with target tissue of the organ and the body-surface electrode is in contact with the skin of the patient.

Using unipolar configuration for irreversible-electroporation (IRE)

A method includes inserting an array of multiple electrodes, fitted at a distal end of a catheter, into a cavity in an organ of a patient. The array is brought into contact with an inner surface of the cavity. An input is received from a user, the input specifying one or more tissue segments to be ablated on the inner surface. In response to the input, using a processor, one or more pairs of the electrodes in the array are selected that, when driven with irreversible electroporation (IRE) signals, would ablate the specified tissue segments. The specified tissue segments are ablated by applying the IRE signals to the pairs of the electrodes.
