Approaches for selecting an electrode combination of multi-electrode pacing devices are described. Electrode combination parameters that support cardiac function consistent with a prescribed therapy are evaluated for each of a plurality of electrode combinations. Electrode combination parameters that do not support cardiac function are evaluated for each of the plurality of electrode combinations. An order is determined for the electrode combinations based on the parameter evaluations. An electrode combination is selected based on the order, and therapy is delivered using the selected electrode combination.

Method and apparatus to perform electrode combination selection

Various aspects of the present subject matter relate to a method. According to various method embodiments, cardiac activity is detected, and neural stimulation is synchronized with a reference event in the detected cardiac activity. Neural stimulation is titrated based on a detected response to the neural stimulation. Other aspects and embodiments are provided herein.

Closed loop neural stimulation synchronized to cardiac cycles

A system for reducing proarrhythmic effects of neural stimulation is provided. One aspect of this disclosure relates to an implantable medical device (IMD) for reducing proarrhythmic effects of neural stimulation. The IMD includes a neural stimulator adapted to deliver an electrical signal through at least one electrode to provide vagal stimulation. The IMD also includes a controller adapted to receive atrial arrhythmia vulnerability data and to control a therapeutic stimulator to limit atrial proarrhythmia using the atrial arrhythmia vulnerability data. The controller is further adapted to receive atrial arrhythmic history data and to control a therapeutic stimulator to limit atrial proarrhythmia using the atrial arrhythmic history data, according to various embodiments. According to one embodiment, the therapeutic stimulator includes the neural stimulator. The therapeutic stimulator includes a cardiac stimulator adapted to provide cardiac rhythm management therapy, according to various embodiments. Other aspects and embodiments are provided herein.

Neural stimulation system for reducing atrial proarrhythmia

Systems and methods for determining pacing timing intervals based on the temporal relationship between the timing of local and non-local cardiac signal features are described. A device includes a plurality of implantable electrodes electrically coupled to the heart and configured to sense local and non-local cardiac signals. Sense circuitry coupled to first and second electrode pairs senses a local cardiac signal via a first electrode pair and a non-local cardiac signal via a second electrode pair. Detection circuitry is used to detect a feature of the local signal associated with activation of a heart chamber and to detect a feature of the non-local signal associated with activation of the heart chamber. A control processor times delivery of one or more pacing pulses based on a temporal relationship between timing of the local signal feature and timing of the non-local signal feature.

Local and non-local sensing for cardiac pacing

A method and apparatus is described for preventing or reducing the ventricular remodeling that normally occurs after a myocardial infarction using pacing therapy. By pacing sites in proximity to the infarct with appropriately timed pacing pulses, the infarct region is pre-excited in a manner that lessens the mechanical stress to which it is subjected, thus reducing the stimulus for remodeling. If capture by the pacing pulses is not being achieved, the device may be configured to change the pacing pulse energy and/or pacing site as appropriate.

Post-MI pacing with autocapture function

In connection with capture detection for a heart chamber with backup pacing in a contralateral heart chamber, a cardiac signal of the first heart chamber is sensed following delivery of a pacing pulse. The cardiac response of the first heart chamber to the pacing pulse is classified based on one or more features of the sensed cardiac signal. A backup pacing pulse is delivered to a second heart chamber contralateral to the first heart chamber. For example, the timing of the delivery of the backup pacing pulse may be based on the expected or detected timing of the features used to classify the cardiac pacing response. The backup pace may be delivered within a detection window used for sensing the features indicative of the cardiac pacing response.

Capture detection with cross chamber backup pacing

An implantable cardiac device is configured to classify cardiac arrhythmias using a plurality of arrhythmia discrimination algorithms. Data is provided that is associated with a plurality of cardiac arrhythmic episodes for which a cardiac electrical therapy was delivered or withheld by the implantable medical device based on the plurality of arrhythmia discrimination algorithms. A metric for each of the arrhythmic episodes is computed. The metric defines a measure by which the implantable cardiac device properly classified the arrhythmia. Potentially misclassified arrhythmic episodes of the plurality of cardiac arrhythmic episodes for which cardiac electrical therapy was inappropriately delivered or withheld are algorithmically identified using the metric.

Method and apparatus for identifying potentially misclassified arrhythmic episodes

Methods and systems are directed to selecting from a variety of capture verification modes. A plurality of capture verification modes, including a beat by beat capture detection mode and a capture threshold testing mode without intervening beat by beat capture detection is provided. An efficacy of at least one of the capture verification modes is evaluated, and based on the evaluation, a capture verification mode is selected.

Methods and systems for selecting capture verification modes

An implantable cardiac function management device including a programmable controller can be used to include a user-specifiable therapy control parameter set. The therapy control parameter set may then be configured to include at least one therapy control parameter that is user-configurable to automatically switch from a first parameter value to a second parameter value at a time that occurs between separate user programming sessions of the device. Various attributes of physiological measures may allow for refinement of the parameter sets to adapt to changed conditions of the subject. Methods of use are also presented.
