A printable wristband form includes a plurality of wristbands cut into a multi-layer sheet of the form. Each wristband includes a release tab formed at an end thereof. A release liner is disposed between layers of the sheet in a single region defined by the release tabs. The form preferably includes adult sized wristbands and infant sized wristbands arranged on the same sheet for use in hospital maternity settings.

An improved identification band such as a wristband is provided with preprinted adhesive labels having a correlated unique identification code on the identification band and each of the labels. A label portion is separable from the wristband portion such that the wristband may be applied to a person or object to be identified. The label portion may be affixed to a specimen vial or other object to be identified as having a correlation with the person or object wearing the wristband. Individual labels are separable from the label portion, and the band portion in certain embodiments, so as to be applicable to other objects to be correlated to the person or object.

An identification bracelet for mounting about a person's wrist or the like incorporates a sealable window to protect wearer-related information against contact with moisture and the like. The bracelet includes a flexible band defining an information-bearing zone, in combination with an overlying adhesive-backed transparent cover strip. In an initial state, the cover strip has opposite ends securely adhered to the band, and a central window segment separated from the band by a peel-off release film. The window segment is adapted for lift-away separation from the band as by tearing along a line of weakness line at one end thereof to expose the information-bearing zone for receiving wearer-related information, and for facilitated peel-off removal of the release film, followed by adhesively seating the strip central window segment onto the band in a manner defining a sealed perimeter overlying and protecting the wearer-related information.
