Disclosed is a surgical instrument directed to phacoemulsification for cataract eye surgery. The instrument generally includes a dual frequency voltage producing circuit comprising a low-frequency voltage pathway with a low-frequency LC Network and a high-frequency voltage pathway with a high-frequency LC network. A phacoemulsification needle extends from a handpiece able to be driven by a piezoelectric transducer. The piezoelectric transducer is electrically connected to the dual frequency voltage producing circuit. The high-frequency voltage pathway is electrically tuned with the physical high natural frequency of the handpiece and the low-frequency voltage pathway is electrically tuned with the physical low natural frequency of the handpiece for efficient transfer of energy.

Phacoemulsification circuit

Disclosed is a surgical instrument for cataract eye surgery. The instrument generally includes a handpiece that delivers sub-ultrasonic and ultrasonic vibrations in either a continuous emulsification mode or with on-off pulses that dynamically drive a hollow needle in either a pulsed fragmentation mode or a pulsed emulsification mode. The pulsed fragmentation mode is efficient at cutting lens tissue and the pulsed emulsification mode is efficient in emulsifying the cut lens tissue. The pulsed modes manage heat buildup from becoming excessive in the eye during the cataract surgery. While in the pulsed fragmentation mode, the hollow needle is never given the chance to vibrate at an established resonant frequency of the handpiece due to the short on-off period. In contrast, the pulsed emulsification mode has a long enough on-off period to permit an ultrasonic resonant frequency in the handpiece to develop, which drives the hollow needle at a higher energy than the pulsed fragmentation mode.

Fragmentation and emulsification modes in a cataract surgical device

Disclosed is a surgical instrument directed to phacoemulsification for cataract eye surgery. The instrument generally includes a hollow titanium needle extending from a vibration generating handpiece. Together, the hollow needle and handpiece form an aspiration pathway to suck cataractous debris from an eye. A piezoelectric transducer in the handpiece generates both high and low ultrasonic frequency vibrations that rings the needle. The low frequency produces a node-free standing wave along the needle and the high frequency produces a standing wave along the needle with a node of minimum amplitude along the needle. Both frequencies produce a high anti-node at the needle's tip. The low frequency causes higher cavitation for emulsifying the cataract and the high frequency facilitates fragmentation of the cataract with a low heat portion of the needle at the eye incision point. The placement of the node along the needle can be tailored by way of a tapered section in a step horn region of the handpiece.

Described is a phacoemulsification device for eye surgery that generally comprises an ultrasonically vibrating aspiration needle that withdraws ocular material from inside of the eye. The ocular material withdrawn from the eye is equally replaced with irrigation fluid provided through irrigation ports in an irrigation sleeve surrounding the aspiration needle. In the event the aspiration needle stops withdrawing ocular material, such as if it becomes occluded with a chunk of ocular material, irrigation fluid is directed to the outside of the eye via at least one irrigation aperture to keep the eye cool where the needle is ultrasonically vibrating.

Occlusion-activated heat suppression infusion sleeve

A surgical instrument and method, for example to accomplish phacoemulsification, are disclosed. The surgical instrument includes a handpiece that includes a piezoelectric transducer. A hollow titanium needle having a substantially cylindrical portion and a free distal tip is attached to the handpiece by way of a threaded supported end structure. The piezoelectric transducer is driven by a circuit to periodically expand and contract at a high-ultrasound frequency that rings the hollow titanium needle with a high-ultrasonic frequency standing wave having a node of minimum amplitude residing in the substantially cylindrical portion between the supported end structure and the free distal tip, and to periodically expand and contract at an ultrasound frequency that rings the hollow titanium needle with an ultrasonic frequency standing wave, the circuit adapted to between the high-ultrasonic frequency and the ultrasonic frequency.

Phacoemulsification ultrasonic device switching between different operational modes

A surgical instrument and method, for example to accomplish phacoemulsification, are disclosed. The surgical instrument includes a handpiece that has a piezoelectric transducer, and a titanium needle having a free distal tip and a supported end that is attached to the handpiece. The titanium needle has a substantially cylindrical portion with an outer diameter in the range 0.5 mm to 1.5 mm and a length in the range 12 mm to 37 mm. A circuit drives the piezoelectric transducer to periodically longitudinally expand and longitudinally contract at a driving frequency. The driving frequency is selected to ring the titanium needle with a standing wave that is characterized by longitudinal expansion and longitudinal contraction, the standing wave having a distal node of minimum amplitude in the substantially cylindrical portion.

A surgical apparatus, used in a phaco procedure, includes a filter assemblyfor near, or complete, elimination of air bubbles in an aspiration line of an aspiration system. The apparatusincludes a suppressor, which, during an aspiration mode of operation, maintains a fluid flow rate at 50 cc/min, and, upon an occlusion breaking free, dynamically decreases the fluid flow rate from a high surge rate to the rate at 50 cc/min. The apparatusincludes facility for operating in a reflux mode to direct unrestricted fluid flow through the aspiration line of the aspiration system

Surgical apparatus and methods asociated therewith

An apparatus and method to suppress vacuum surges in a surgical aspiration system is disclosed and claimed. A vacuum surge suppressor includes a first fluid path for coupling to a surgical instrument, and a filter attached to the first fluid path. A flow restrictor is coupled to the filter with the filter disposed upstream of the flow restrictor. The filter defines a filter longitudinal axis that may be oriented vertically with respect to gravity, so that a downstream direction at the filter is upwards. The vacuum surge suppressor also includes a second fluid path for coupling to a vacuum pump. The second fluid path is connected to the flow restrictor and disposed downstream of the flow restrictor. A third fluid path is coupled to the filter and is connected to the second fluid path, bypassing the flow restrictor. A valve in the third fluid path selectively obstructs or permits flow in the third fluid path.

Vacuum surge suppressor for surgical aspiration systems

An aspiration tube has a relatively high fluidic resistance. The tube is coupled to a medical device and a pump of an aspiration system. The tube can be designed to create a pressure drop that at least equals the maximum vacuum pressure of the pump. The fluidic resistance can be accomplished with a tube having an inner diameter less than 0.05 inches. The aspiration tube and an irrigation tube may be coupled to an anterior chamber of a cornea during a phaco procedure. The aspiration tube is sized so that even if a maximum pressure occurs, the resultant aspiration flowrate will be such that the cornea will not be damaged when an occlusion is cleared from the tube. An in-line filter may also be attached to the aspiration tube to filter out particles in the system.

Aspiration system for medical devices

An apparatus and method to suppress vacuum surges in a surgical aspiration system is disclosed and claimed. A vacuum surge suppressor includes a first fluid path for coupling to a surgical instrument, and a filter attached to the first fluid path. A flow restrictor is coupled to the filter with the filter disposed upstream of the flow restrictor. The vacuum surge suppressor also includes a second fluid path for coupling to a vacuum pump. The second fluid path is connected to the flow restrictor and disposed downstream of the flow restrictor. The second fluid path defines a path internal pressure. A third fluid path is coupled to the filter and is connected to the second fluid path, bypassing the flow restrictor. A valve in the third fluid path obstructs flow in the third fluid path in response to the path internal pressure.

An ophthalmic aspiration system that can be used with a hand piece and a vacuum source, is disclosed and claimed. The aspiration system includes a first tube that is connected to the hand piece and a second tube that is connected to a vacuum source. A filter assembly is connected to both tubes to filter out particles aspirated into the system. The second tube has an inner diameter smaller than an inner diameter of the first tube. The smaller second tube limits the amount of flow through the system to minimize vacuum surges caused by occlusions.

Aspiration system for ophthalmic medical devices

A control circuit that provides a driving signal to a transducer coupled to a mechanical cutting element. The transducer is capable of operating in a resonant mode. The driving signal contains a plurality of pulses provided in a time interval that does not cause the transducer to operate in the resonant mode.
