A heart vent catheter includes an elongate, flexible tube that defines a first passageway. The catheter has a distal end, a central portion, and a proximal end. A balloon is included as part of the distal end and surrounds at least a portion of the tube. A plurality of first openings are formed in the tube adjacent the balloon on the forwardmost side of the balloon. A plurality of second openings can be formed in the tube adjacent the balloon on the proximal side of the balloon. After the distal end has been inserted into a desired part of a patient's heart, such as the left ventricle, the balloon can be inflated so as to engage a part of the heart such as the mitral valve and thereby prevent undesired withdrawal of the catheter. The proximal end includes a formation that can be connected to a vacuum source. The openings in the tube in the distal end permit blood, other fluid, debris and/or air to be withdrawn from the heart through the first passageway under suction. After the surgical procedure has been completed, the balloon can be collapsed and the catheter can be withdrawn from the heart.

Heart vent catheter and method of use

A method and apparatus for the amelioration of sleep apnea and other obstructive breathing disorders in a patient includes a collar configured to encircle the patient's neck, and a jaw-engagement mechanism carried by the collar. The jaw-engagement mechanism includes a pad configured to contact the underside of the patient's lower jaw in the area of the anterior triangle behind the mandibular symphysis and to be received in a small hollow of the patient's jaw. In use, the pad can apply force against a surface of the patient's lower jaw in order to push it forwardly and upwardly. The pad preferably is secured to a movable post that can be positioned such that a desired amount of force is applied to the patient's lower jaw.

Amelioration of sleep apnea and other obstructive breathing disorders

A device for excluding the left atrial appendage (LAA) includes first and second members, each with first and second ends. The second ends are connected by a hinge. The first ends interlock to close the device about the LAA. The members are shaped such that they can exclude the LAA without causing the development of necrotic tissue. If desired, a flexible cover can be provided and the first and second members can be disposed within the cover. If such a cover is used, it will contact the LAA in order to cushion the contact between the members and the LAA and minimize the tendency of the LAA to bleed. The invention also includes methods for using the device to exclude the LAA.

Exclusion of the left atrial appendage

A device for occluding a conduit includes first and second members, each with first and second ends. The second ends are connected by a hinge. The first ends interlock to close the device about the conduit. The members are shaped such that they can occlude the conduit without causing the development of damaged or necrotic tissue. If desired, a flexible cover can be provided and the first and second members can be disposed within the cover. If such a cover is used, it will contact the conduit in order to cushion the contact between the members and the conduit, minimize the tendency of the conduit to bleed, and increase traction relative to the conduit. The invention also includes methods for using the device to occlude a conduit.

Vascular and intestinal occlusion

A method and apparatus for sleep apnea prevention includes a clamp that has an upper clamp segment having a distal end and a hinge end, the distal end being is adapted to engage the patient's upper lip adjacent the nose and a lower clamp segment having a distal end and a hinge end, the distal end being adapted to engage the patient's lower jaw behind the mandibular symphysis. A hinge connects the upper and lower segments at the hinge ends. The upper and lower segments apply force to the patient's face and jaw such that the lower jaw is moved upwardly and forwardly. Adjustable force can be applied by elastic members connected to the upper and loser segments. Alternatively, the hinge can be held in a desired position by a hand-adjustable nut and bolt. The distal end of the upper clamp segment can include portions that project laterally and upwardly in order to apply compressive force to a patient's face on either side of the patient's nose.

Sleep apnea prevention

A method and apparatus for preventing sleep apnea employs an external clamp that has a first pad that engages the upper lip beneath the nose and a second pad that engages the lower jaw on its underside, behind the mandibular symphysis. The pads are connected to each other by a flexible member, or brace. Preferably the second pad is rounded on that portion that contacts the lower jaw. The clamp is held in place by one or more straps, braces or springs that pass about the patient's head. The first pad is held such that it presses against the upper lip while the second pad closes the lower jaw and pulls it forwardly and upwardly using relatively small forces.

A single-arm stabilizer having suction capability includes a single, small leg through which suction can be applied. The leg has an upper surface and a lower surface that are spaced apart to define a chamber. The lower surface includes a plurality of openings that are disposed adjacent each other. A support arm is connected to the leg in order to position the leg as desired. A suction line is in fluid communication with the chamber in order to create a vacuum within the chamber. The stabilizer can be used to stabilize any desired portion of a patient's body, but is particularly effective at stabilizing the septum of a patient's heart. Such stabilization can be accomplished by inserting the leg into the patient's heart through the aorta.

Single-arm stabilizer having suction capability

A surgical retractor includes a pair of small grips that are disposed at the ends of arms that can be moved toward or away from each other. The grips engage and separate the sides of an incision that has been made in a patient. A lifting mechanism that can be attached to one of the arms enables one side of the incision to be raised relative to the other side of the incision. The lifting mechanism includes a clamp that can be connected to the retractor and a foot that is connected to the clamp and which is movable relative thereto. The foot is engageable with the patient such that extension of the foot causes a selected grip to be raised. Retraction of the foot permits the selected grip to be lowered. In the preferred embodiment, the foot is disposed at one end of a notched rack; a rotatable pinion is in contact with the notches to extend and contract the rack; and a pivotally movable pawl permits movement of the rack in one direction and prevents movement of the rack in the other direction.

Surgical retractor having lifting capability

A clamp is provided for attachment to the left atrial appendage (LAA). The clamp can be locked in a closed position so as to remain in place on the LAA. After being attached to the LAA, the clamp can be moved to a desired position such that the LAA can be retracted to a desired extent. The clamp can be held in any desired position, preferably by being connected to a thoracic retractor that is being used to retract the patient's sternum or ribs. A movable member is connected between the clamp and a stationary portion of the retractor. By locking the movable member in a fixed position relative to the stationary portion, the clamp can be maintained in a desired position and the LAA can be retracted as desired.

Retraction of the left atrial appendage

Spinal arthrodesis is accomplished by implanting an intervertebral spacer made of a biocompatible material that promotes tissue ingrowth and bony fusion. The spacer is implanted into a graft bed formed between adjacent vertebral bodies. The spacer includes a body portion having anterior and posterior end faces. Opposed flanges project outwardly from the body portion at axial locations adjacent the anterior and posterior end faces. The flanges taper radially outwardly to an outer edge that is sharp enough to penetrate vertebral end plates. After a graft bed has been formed, and after the vertebral bodies have been distracted, the spacer is inserted into the graft bed and positioned such that the flanges are adjacent the vertebral end plates. Upon release of the distraction, the vertebral bodies will move toward each other and the flanges will be pressed into the end plates. The effect of the pressing action is to secure the spacer tightly in place within the graft bed such that migration of the spacer is prevented.

Arthrodesis of vertebral bodies

A technique to manufacture small joint orthopedic implants includes the steps of taking standard radiographs of a pathologic joint and the corresponding non-pathologic joint. In order to provide an accurate frame of reference, a specialized marker is placed in the radiographic field. By inspection of the radiographs and by comparison with the marker, the dimensions of the cortical bone and the cancellous bone can be quickly and accurately determined. These dimensions can be used to manufacture a suitable implant and installation tool. Typically, the implant will include a stem from which a post projects. A radially extending collar is located at the intersection between the stem and the post. A mating head is attached to the post. The head closely approximates the size and shape of the natural head being replaced. The stem will be non-round in cross-section to prevent rotation of the stem in the bone. For many applications, the head will not be fixedly attached to the post, but will be rotatable about the longitudinal axis of the post. One or more spacers that fit about the stem also can be provided in order to adjust the distance that the head projects from the bone.

Small joint orthopedic implants and their manufacture

A surgical marker/connector can be provided in the form of a ring made of a fluoroscopic material, including a shape memory alloy such as NITINOL. The ring preferably includes a plurality of small suture attachments in the form of loops that are disposed on the periphery of the ring and which lie in a plane that includes the ring itself. The loops are large enough to receive sutures which, in turn, can be used to connect the ring to a portion of a patient's body, or to connect separate portions of a patient's body to each other using the marker/connector as an intermediate connector. If the ring is made of a shape memory alloy, the ring can be wrapped tightly about an elongate member such as a mandrel, which than can be inserted into the patient's body through a trochar or cannula. After insertion, the ring will change temperature and resume its original configuration where it can be sutured in place as desired. If the ring is not made of a shape memory alloy, the ring can be inserted into the patient's body by conventional surgical techniques and then sutured in place as desired.
